Cracked Heels Are a Pressure Problem Long Before They Become a Moisture Problem

Everyone reaches for a heel balm when the skin splits. But cracked heels begin with pressure and friction, not dryness. The moisture problem arrives later, after the damage is already structural. Understanding what actually causes callus buildup and skin fissures changes how you treat them, and whether the treatment works at all.
The crack starts from below, not the surface
Dryness makes this worse, but dryness alone on a heel that is not under excessive pressure rarely produces deep fissures. You can apply the richest shea butter available and still have cracked heels if the mechanical load is not addressed. The balm softens the callus temporarily; the pressure rebuilds it within days.
What actually builds callus in the first place
Three things drive abnormal callus formation at the heel:- Open-backed footwear. Chappals, slides, and kolhapuri sandals, worn through most of the year across India, give the fat pad no lateral containment. It spreads freely, and the rim skin stretches with every step. The skin thickens in response.- Hard flooring. Marble, granite, and cement floors, standard in most Indian homes, transmit force directly. There is no absorption between the foot and the surface.- Body weight distribution. Pronation, the inward roll of the foot, shifts load to the inner heel rim. Supination shifts it to the outer edge. Either pattern concentrates pressure unevenly, and the callus builds where the concentration is highest.
Moisture enters the picture because thickened callus is poor at retaining water. It dries faster than normal skin, cracks more easily, and loses the flexibility that would let it absorb the next impact. But the callus came first. The dryness is a consequence, not the origin.
Why standard heel care often fails
A 2019 study published in the Journal of the American Podiatric Medical Association found that patients with plantar callus who received only topical emollient treatment showed significantly less improvement than those who combined emollients with offloading, meaning pressure redistribution through insoles or footwear changes. The callus returned in the emollient-only group within six to eight weeks.
The mechanical load was not touched. The skin responded accordingly.
Two specific failure patterns:1. Pumicing wet callus. Scrubbing softened callus removes surface thickness but leaves the deeper, load-bearing layers intact. The skin re-thickens to the same depth within weeks because the stimulus, pressure, is unchanged.2. Applying balm to fissured skin without closing the fissure first. An open crack cannot be healed from the outside in. A liquid bandage or medical-grade heel tape closes the fissure mechanically, allowing the skin beneath to regenerate. Balm on an open crack moisturises the wound edges but does not pull them together.

The heel is not fragile. It is one of the most load-tolerant structures in the body. But it has a threshold, and most cracked-heel problems are a sign that threshold has been exceeded repeatedly, over months, without the load being adjusted.
Balm is not wrong. It is just the last step being applied first, which is why the crack keeps coming back.
