Those small, rough bumps clustered on your upper arms are not a hygiene problem. Keratosis pilaris is a skin condition driven by excess keratin, and the scrubbing most people reach for makes the irritation worse. Here is what the bumps actually are, why they resist soap and water, and what genuinely helps your skin.
What Those Bumps Actually Are
A 2016 review published in the American Journal of Clinical Dermatology estimated that keratosis pilaris affects roughly 40 percent of adults and up to 80 percent of adolescents worldwide, making it one of the most common benign skin conditions that goes almost entirely untreated, or treated incorrectly. The condition runs in families, tends to worsen in dry weather, and has no single definitive cure. What it does have is a clear mechanism, and once you understand the mechanism, the wrong treatments become obvious.
Why Scrubbing Is the Wrong Answer
Friction also activates the skin's repair response, which produces more keratin. You are, in effect, signalling the skin to make more of exactly what is causing the problem. This is why people who scrub daily often report that their bumps feel worse in winter or after a hot shower, both strip skin barrier function and leave the follicle exposed to further irritation.
What Actually Reduces Keratosis Pilaris
Lactic acid and urea are the most studied. Lactic acid, an alpha hydroxy acid, loosens the bonds holding the keratin plug in place and allows it to shed gradually. A concentration of 10 to 12 percent lactic acid, applied once daily after bathing, is the standard starting point in dermatology. Urea at 10 to 20 percent does a similar job while also drawing moisture into the skin. Both are available over the counter in Indian pharmacies under brands like Lacto Calamine's urea range, CeraVe, and various generic formulations. Neither requires a prescription.
Salicylic acid works too, though it is better suited to oilier skin types since it can be drying. Retinoids, vitamin A derivatives, can help by normalising the rate at which skin cells turn over, but they require patience: visible improvement takes eight to twelve weeks and the skin may purge before it improves.
The emollient half of the equation matters as much as the exfoliant. Applying a thick, fragrance-free moisturiser immediately after bathing, within three minutes, while the skin is still slightly damp, seals water into the skin and keeps the follicle environment less hostile to normal shedding. Coconut oil, widely used in Indian households, provides some occlusion but lacks the urea or lactic acid content that actively addresses the plug. It is a reasonable supplement, not a replacement.
The Indian Climate Factor
The practical adjustment is simple: run a humidifier in the bedroom if air conditioning runs overnight, switch from hot showers to lukewarm ones, and double the frequency of moisturiser application during dry months. Fragrance-heavy body lotions, popular in Indian personal care aisles, are worth replacing with fragrance-free options, fragrance is among the most common skin irritants and compounds the inflammation already present in keratosis pilaris.
What to Expect and What to Accept
A dermatologist can prescribe stronger topical retinoids or in-office treatments like laser therapy for persistent redness, but most people see meaningful improvement with consistent over-the-counter lactic acid or urea application over two to three months. The timeline is slow. The skin does not respond to urgency.
The bumps that look like they need harder scrubbing are, in fact, asking for the opposite: less friction, more moisture, and a chemical exfoliant that dissolves the plug from within rather than trying to sand it off from outside. Keratosis pilaris is the skin doing something it was always going to do, producing keratin at a slightly higher rate than the follicle can clear. The only productive response is to work with that biology, not against it.
