Why Tranexamic Acid Alone Hits a Wall
Tranexamic acid blocks the interaction between keratinocytes and melanocytes, essentially interrupting the signal that tells your skin to produce more pigment. In a 2020 study published in the Journal of the American Academy of Dermatology, oral tranexamic acid at 250 mg twice daily showed a 49% reduction in MASI (Melasma Area and Severity Index) scores over 12 weeks. Topical formulations at 3 to 5% concentrations show similar early promise. The problem is that melanin production has multiple triggers, UV exposure, hormonal fluctuation, inflammation, and tranexamic acid addresses only one pathway. The Niacinamide Partnership
Niacinamide does something tranexamic acid cannot: it blocks the transfer of melanin from melanocytes to skin cells. The two mechanisms sit at different points in the same cascade, which is why dermatologists frequently combine them. A 5% niacinamide serum applied before or after a tranexamic acid formulation covers both the production signal and the distribution step. For Indian skin tones, which tend toward Fitzpatrick types III to V and are more prone to post-inflammatory hyperpigmentation, this dual block matters. Niacinamide also reduces redness and strengthens the barrier, which means the combination is gentler than actives that work by exfoliation alone.
Kojic Acid and the Tyrosinase Angle
Tyrosinase is the enzyme that converts tyrosine into melanin. Kojic acid inhibits it directly. Tranexamic acid does not touch tyrosinase. This is why a formulation containing both, or a routine that layers a kojic acid product alongside tranexamic, can reach parts of the pigmentation process that either ingredient misses on its own. Kojic acid at concentrations between 1% and 4% is commonly prescribed by dermatologists treating melasma in India, often in combination serums or as part of a compounded cream. The trade-off: kojic acid can cause contact dermatitis in sensitive skin, so patch testing before committing to a full-face routine is standard protocol.
Where Tretinoin and Sunscreen Fit
Tretinoin accelerates cell turnover, pushing pigmented cells toward the surface faster so they shed. This does not reduce melanin production directly, but it shortens the time pigment spends visible in the skin. When tretinoin is added to a tranexamic and niacinamide routine, the combination targets production, transfer, and clearance, three distinct steps. The catch is that tretinoin increases photosensitivity, which means sunscreen moves from optional to non-negotiable. Dermatologists treating melasma in Indian climates consistently name inadequate sunscreen use as the single biggest reason treatment fails. A broad-spectrum SPF 50+ PA+++ sunscreen, reapplied every two hours during outdoor exposure, is not an add-on to the regimen, it is the regimen's load-bearing wall. Without it, UV exposure re-triggers the pigmentation signal that tranexamic acid is trying to suppress, and the entire stack of actives loses ground daily.
Oral vs. Topical Tranexamic: What Dermatologists Actually Prescribe
In India, oral tranexamic acid for melasma is prescribed off-label, typically at 250 mg once or twice daily for three to four months. It reaches dermal melanocytes more efficiently than most topical formulations, which is why dermatologists use it for deeper, more refractory melasma. Topical tranexamic at 3 to 5% is preferred for maintenance and for patients who cannot take the oral form. The two are sometimes used simultaneously in the early phase, then tapered to topical alone. Neither form is used without the surrounding regimen, sunscreen, a barrier-supporting moisturiser, and at least one complementary depigmenting agent, because the evidence for monotherapy, oral or topical, consistently shows a plateau that combination protocols do not.
Melasma is a chronic condition, which means the goal of treatment is management, not cure. What the pairing logic reveals is that tranexamic acid is a strong opening move in a longer game, it quiets one signal quickly, which buys time for the slower-acting agents to work. The moment the surrounding regimen is removed, the remaining triggers take over. The molecule is not stalling because it is weak; it is stalling because the skin has more routes to pigmentation than one ingredient can cover.
