Iodised salt was supposed to solve iodine deficiency for good. But millions of Indians still show symptoms, fatigue, weight gain, brain fog, without knowing why. The problem is rarely the salt itself. It's everything happening before and after it reaches your food. Here's what your body may already be telling you.

Your thyroid is working, but not well enough

The thyroid gland needs iodine to produce T3 and T4 hormones. When iodine supply drops even slightly below what the gland requires, it compensates by enlarging, a condition called goitre, or by slowing hormone output without enlarging at all. That second pattern, subclinical hypothyroidism, is the one most people miss. Thyroid function tests come back borderline. The doctor says watch and wait. But the body is already rationing hormone production, and the effects accumulate quietly over months.
A 2019 study published in the Indian Journal of Endocrinology and Metabolism found that subclinical iodine insufficiency remains prevalent in several Indian states despite decades of universal salt iodisation programmes, with women of reproductive age showing the highest rates of insufficiency. The salt was there. The iodine, by the time it reached the body, sometimes wasn't.

Fatigue that sleep does not fix

Thyroid hormones regulate how efficiently cells convert fuel into energy. When iodine is low and thyroid output drops, cellular metabolism slows across the board. The result is a fatigue that feels different from ordinary tiredness, it sits in the muscles, makes concentration difficult, and does not lift after a full night's sleep. Many people in this state reach for more chai, more coffee, more sugar, and feel marginally better for an hour before the heaviness returns.
The confusing part is that standard blood panels don't always catch it. TSH levels within the normal range don't rule out tissue-level iodine shortage. The symptom, persistent, unresponsive fatigue, is often the earliest and most consistent signal the body sends.

Unexpected weight gain and a slowing metabolism

Iodine deficiency slows metabolism through the thyroid axis, but the weight gain it produces has a specific character. It tends to be puffy rather than fatty, fluid retention around the face, hands, and ankles. The skin can feel slightly thicker or dougher to the touch. This is myxoedema in its mild form, caused by glycosaminoglycan deposits under the skin when thyroid hormone levels fall.
In Indian households where cooking happens largely in stainless steel or non-stick pans, iodised salt is often added early in the cooking process. Heat degrades iodine. A 2012 study in the journal Food and Nutrition Bulletin found that up to 50 percent of iodine content in salt can be lost during cooking, particularly when salt is added to hot oil or boiling water at high temperatures. The portion of iodine actually absorbed may be a fraction of what the packet suggests.

Brain fog, slow thinking, and memory gaps

Iodine is not only a thyroid nutrient. The developing brain depends on it heavily, but adult brain function is also sensitive to iodine status. Low thyroid hormone output affects the speed of neural transmission and the efficiency of memory consolidation. People describe it as thinking through gauze, words take longer to come, tasks that once felt automatic require deliberate effort, and short-term recall becomes unreliable.

This symptom is frequently attributed to stress, screen fatigue, or ageing. Those causes are real. But when brain fog appears alongside unexplained weight changes and persistent fatigue, iodine status is worth checking directly through a urinary iodine concentration test, which is more accurate than serum iodine for assessing population-level and individual sufficiency.

Hair thinning and dry, flaky skin

Hair follicles are among the fastest-cycling cells in the body and among the first to suffer when thyroid hormones drop. Iodine deficiency-driven hypothyroidism causes hair to thin diffusely, not in patches, but across the entire scalp, including the outer third of the eyebrows, which is a specific clinical marker. The skin simultaneously loses its ability to retain moisture, turning dry, rough, and prone to flaking even in humid climates.
In coastal Indian states where fish and seafood are dietary staples, iodine intake from food sources supplements what salt provides. Inland populations, particularly in the Gangetic plains and hilly regions historically called the goitre belt, have historically relied almost entirely on iodised salt as their iodine source. For vegetarians and vegans who avoid seafood, dairy, and eggs, all secondary iodine sources, the margin between sufficiency and deficiency is narrow.

The body does not store large iodine reserves. Daily intake matters. And the gap between the iodine on the label and the iodine that reaches the bloodstream, lost to heat, to storage in transparent containers under sunlight, to the simple chemistry of an unstable compound, is wider than most people assume. Iodised salt is a delivery system. Whether it delivers depends on how you store it, when you add it, and what else your diet contains.
