Start Tracking Calcium Like You Track Everything Else
The average Indian woman gets roughly 400 mg of calcium a day from food. She needs 1,000 mg. That gap doesn't show up on any test until the bone is already thinner. Dairy is the obvious fix, but absorption is the real variable. Ragi, finger millet, delivers about 344 mg of calcium per 100 g and has been a staple in Karnataka and Andhra Pradesh for generations, not as a supplement but as a meal. A bowl of ragi mudde or ragi dosa does more for bone calcium than a glass of low-fat milk, partly because it comes with magnesium, which helps the calcium actually enter bone tissue.
The 2019 Indian Council of Medical Research (ICMR) Nutrient Requirements report flagged calcium deficiency as one of the most widespread and under-addressed nutritional gaps in Indian women over 35. Getting to 1,000 mg through food, not supplements alone, requires deliberate daily choices: sesame seeds in your sabzi, amaranth flour in your roti, a small serving of low-fat curd at lunch. The body absorbs calcium from food more efficiently than from tablets, and it doesn't dump the excess the way kidneys must when a supplement dose is too high.
Get Your Vitamin D Checked, Not Just Assumed
India has more than 300 sunny days a year in most cities, and Indian women are still among the most vitamin D-deficient populations globally. A 2014 study published in the Journal of Mid-life Health found that 91% of perimenopausal Indian women had insufficient vitamin D levels. The reasons are practical: long sleeves, staying indoors during peak sun hours, darker skin tones requiring longer sun exposure to synthesise the same amount, and a diet that rarely includes fatty fish or fortified foods in meaningful quantities.
Vitamin D is what tells the gut to absorb calcium. Without adequate D, even a calcium-rich diet leaves bones undernourished. The fix is a 25-OH vitamin D blood test, not a guess. If levels are below 30 ng/mL, supplementation, typically 1,000 to 2,000 IU daily, is the clinical recommendation, but the dose should be confirmed with a doctor because vitamin D toxicity, while rare, is real. Fifteen to twenty minutes of midday sun on the forearms and face, three to four times a week, helps maintain levels once they're corrected.
Lift Something Heavy, Regularly
Bone is living tissue. It responds to mechanical load by building density, this is the principle behind weight-bearing exercise, and it is one of the few inputs that directly stimulates osteoblasts, the cells that lay down new bone. Walking is good. Resistance training is better. A 2015 meta-analysis in the Journal of Bone and Mineral Research confirmed that progressive resistance exercise significantly increased lumbar spine and femoral neck bone mineral density in pre- and perimenopausal women.
This doesn't require a gym. Bodyweight squats, lunges, stair climbing with a loaded bag, and resistance band exercises all count. The key word is progressive, the load has to increase over time, or the bone stops receiving a signal to adapt. Women who have never lifted weights often see the fastest early gains, which makes the pre-menopause window particularly valuable. Starting at 38 or 42 is not too late. Starting at 55, after significant bone loss, is harder.
Cut the Two Habits That Quietly Strip Bone
Smoking and heavy alcohol consumption both reduce bone density through separate mechanisms. Nicotine directly inhibits osteoblast activity and reduces estrogen levels, which accelerates the bone loss that menopause will already trigger. Alcohol interferes with calcium absorption and suppresses the liver's ability to activate vitamin D. A woman who smokes and drinks regularly is compounding two separate bone-depleting processes on top of the hormonal changes that come with perimenopause.
Salt is the less-discussed third factor. High sodium intake increases the amount of calcium excreted in urine. A diet heavy in processed foods, pickles, papad, and packaged snacks, all high in sodium, can quietly drain calcium reserves even when intake looks adequate on paper. Reducing sodium doesn't require eliminating Indian cooking; it means being deliberate about packaged and preserved foods, which are where the sodium load actually concentrates.
Protect Estrogen While You Still Have It
Estrogen is the primary hormonal brake on bone resorption, the process by which the body breaks down old bone. As estrogen declines in perimenopause, that brake weakens, and bone resorption outpaces bone formation. This is why women lose bone faster in the five years around menopause than at any other point in their lives. A 2020 paper in Bone Reports noted that Indian women tend to enter menopause about two years earlier than their Western counterparts, at a median age of around 46, which means the window for building peak bone mass is shorter.
Phytoestrogens, plant compounds that weakly mimic estrogen, may offer partial support during this transition. Soy, flaxseed, and sesame contain isoflavones and lignans that bind estrogen receptors at low levels. The evidence is not conclusive enough to treat phytoestrogens as a medical intervention, but incorporating soy milk, tofu, or a tablespoon of ground flaxseed daily is a reasonable dietary choice with no downside. For women with significant symptoms or a family history of osteoporosis, hormone replacement therapy remains the most evidence-backed option and deserves a direct conversation with a gynaecologist rather than avoidance based on outdated fears.
Build the Habit of Knowing Your Numbers
A DEXA scan measures bone mineral density and produces a T-score that tells you exactly where your bones stand relative to peak bone mass. Most Indian women have never had one. The standard recommendation is to get a baseline scan at menopause, but for women with risk factors, early menopause, low body weight, a family history of fractures, long-term steroid use, or a history of eating disorders, a scan in the mid-40s gives actionable information while there is still time to change the trajectory.
The T-score that defines osteoporosis is -2.5 or below. Osteopenia, the stage before osteoporosis, sits between -1.0 and -2.5. Catching bone loss at the osteopenia stage means lifestyle and dietary interventions can still make a meaningful difference. Catching it at osteoporosis means the conversation shifts to medication. The scan itself takes about 15 minutes and involves minimal radiation, less than a chest X-ray.
Bone density at 60 is largely determined by what happened between 30 and 50. The habits that protect bone are not dramatic or expensive, but they require years of consistency to work. A fracture at 65 that takes six months to heal and permanently limits mobility is the cost of a decade of neglect. The DEXA number is just a way of seeing, clearly and early, what the body has been quietly recording all along.
