Delivery riders clock 10-hour shifts on Indian roads through pollution, traffic, and extreme heat, yet no platform runs a single health screening before they start. These eight risks accumulate silently, and by the time symptoms appear, the damage is already months old.

Musculoskeletal damage from continuous vibration

Every hour on a two-wheeler transmits road vibration directly through the spine. A 2019 study published in the International Journal of Industrial Ergonomics found that professional two-wheeler riders exposed to whole-body vibration for more than four hours daily showed significantly higher rates of lumbar disc degeneration than non-riding workers. Indian delivery riders routinely exceed that threshold on roads that mix potholes, speed breakers, and broken asphalt. The lower back absorbs most of it. Riders rarely connect the chronic ache to the job until the disc has already compressed.


Chronic respiratory damage from roadside pollution

Riders breathe at the exhaust level of the vehicles in front of them for the entire shift. The Central Pollution Control Board has recorded PM2.5 levels in cities like Delhi, Kanpur, and Patna that exceed WHO safe limits by factors of 10 to 20 during peak traffic hours. A thin cloth mask, the standard self-purchased protection for most delivery riders, filters almost none of it. Long-term PM2.5 exposure at these concentrations is linked to reduced lung capacity, chronic bronchitis, and elevated cardiovascular risk. No platform currently requires a baseline spirometry test at onboarding.


Heat stress and dehydration

A rider working a noon-to-3pm slot in May in Hyderabad or Nagpur is operating in ambient temperatures that regularly cross 42°C, on asphalt that radiates additional heat upward. The Indian Journal of Occupational and Environmental Medicine has documented heat exhaustion as a recurring occupational hazard among outdoor workers in the Deccan plateau region. Riders are disincentivized from stopping to drink water because every pause costs delivery time and, on incentive-based pay structures, directly cuts earnings. Chronic mild dehydration at this level impairs kidney filtration over months.


Hearing loss from traffic noise

The permissible occupational noise exposure limit set by the Factories Act is 90 decibels over an eight-hour shift. Traffic noise in Indian metro corridors regularly hits 85 to 95 decibels at street level, with horns spiking well above that. Riders who wear earphones for navigation add another layer of sound directly into the ear canal. Noise-induced hearing loss is gradual and painless until it isn't. No gig platform has ever published a hearing screening protocol for its rider workforce.



Eye damage from UV exposure and particulate matter

Riders without full-face helmets spend hours with their eyes exposed to direct UV radiation and airborne particulate matter. Prolonged UV exposure without protection accelerates pterygium formation, a growth on the conjunctiva common among outdoor workers in tropical climates. Studies from the All India Institute of Medical Sciences have flagged outdoor workers in high-UV Indian regions as a high-risk group for early-onset cataracts. A delivery rider's eyes are essentially unprotected occupational equipment.


Fatigue-related cognitive impairment

Fatigue is not just tiredness. After six consecutive hours of traffic navigation, the cognitive load of constant hazard detection, route switching, and time pressure produces decision-making impairment comparable to mild intoxication, according to research from the National Safety Council (US). Indian roads add variables, cattle crossings, wrong-way vehicles, sudden pedestrian movement, that compound the cognitive demand. Riders in the final hours of a long shift are making split-second decisions with a brain that has already spent its reserves. No platform screens for fatigue accumulation or mandates rest intervals.



Wrist and hand injuries from handlebar stress

Sustained gripping of handlebars for hours compresses the ulnar nerve at the wrist, producing a condition called handlebar palsy, weakness and numbness in the ring and little fingers. It is well-documented in long-distance cyclists but occurs in delivery riders too, particularly those navigating stop-start urban traffic where constant brake and throttle adjustments repeat the same micro-movement thousands of times per shift. Most riders attribute the numbness to sleeping positions and never report it.


Mental health deterioration from isolation and income instability

A 2022 survey by the Fairwork India project, which monitors gig economy working conditions across Indian platforms, found that a majority of surveyed delivery riders reported high stress levels linked to income unpredictability, customer ratings pressure, and lack of any formal grievance mechanism. Chronic occupational stress at this level is associated with anxiety disorders, disrupted sleep, and elevated cortisol, which itself raises cardiovascular risk. The physical and psychological risks are not separate categories. They compound each other across a shift, and across years.



The eight risks above are not random, they are all products of the same structural gap: a workforce classified as independent contractors rather than employees, which means no employer is legally obligated to run pre-employment health checks, provide protective equipment, or monitor occupational exposure. The body keeps the record the platform never asks for.
