Walk into almost any gym in Mumbai or Bengaluru and ask a trainer about creatine. A significant number will lower their voice, shake their head, and say something like "bhai, woh toh steroids hain." This is factually incorrect, and the confusion has real costs.
Creatine is a compound the human body produces naturally, primarily in the liver and kidneys, from three amino acids: arginine, glycine, and methionine. The body makes roughly one gram per day. Red meat and fish supply additional amounts through diet. A creatine supplement does nothing more than increase what your muscle cells already store. Anabolic steroids are synthetic derivatives of testosterone that alter hormone levels. The two are not in the same biological category, not even close.
The steroid label has stuck partly because creatine became visible in Indian gyms around the same time doping scandals were making international headlines. Association did the rest of the damage.
What the Research Actually Shows
The International Society of Sports Nutrition published a position stand, one of the most cited documents in exercise science, concluding that creatine monohydrate is the most effective ergogenic nutritional supplement available for increasing high-intensity exercise capacity and lean body mass. That paper has been updated multiple times and the conclusion has not changed.
A 2021 meta-analysis published in the Journal of Strength and Conditioning Research, covering over 22 randomised controlled trials, found that creatine supplementation consistently improved performance in short-burst, high-intensity activities: sprinting, weightlifting, and repeated jump efforts. The average strength gain across studies was roughly 8 percent above placebo groups over six to twelve weeks.
For Indian athletes specifically, a 2019 study from the Sports Authority of India's research wing found that vegetarian athletes, a large portion of the Indian fitness population, had significantly lower baseline muscle creatine stores than omnivores. This matters because vegetarians do not get dietary creatine from meat. Supplementation for this group is not a luxury; it addresses a genuine dietary gap.
The Kidney Damage Fear
The most persistent myth in Indian gym culture is that creatine destroys kidneys. This one has a grain of origin: creatine metabolism produces creatinine as a byproduct, and creatinine is a standard marker doctors use to assess kidney function. When someone on creatine gets a blood panel, their creatinine reads slightly elevated. Doctors unfamiliar with supplementation sometimes flag this as a warning sign.
The elevation is a measurement artifact, not organ damage. Multiple long-term studies, including a 2019 review in the Journal of the International Society of Sports Nutrition, found no adverse effect on kidney function in healthy individuals taking standard doses of three to five grams per day over periods extending to five years. The caveat is real: people with pre-existing kidney disease should not supplement creatine without medical supervision. For healthy adults, the fear is not supported by evidence.
Why Indian Fitness Culture Gets This Wrong
Three things converge to keep the myths alive. First, supplement literacy in India is low relative to training culture. Gym-goers learn from trainers, trainers learned from older trainers, and misinformation passes down without a research correction entering the chain.
Second, the supplement market in India has historically been flooded with adulterated products. A 2020 study by the Food Safety and Standards Authority of India found that a significant percentage of protein and supplement products sampled from the market contained undeclared ingredients or incorrect labelling. This is a real problem, but it is an adulteration problem, not a creatine problem. Blaming the compound for the supply chain's failures is a category error.
Third, Indian dietary culture, particularly in communities where vegetarianism is the norm, has no food-based familiarity with creatine. Whey protein at least maps onto paneer and dahi in the cultural imagination. Creatine has no such anchor, so it lands as chemical and foreign.
How to Use It Correctly
Creatine monohydrate is the form with the most research behind it. Three to five grams per day, taken consistently, is the standard effective dose. Loading phases, where users take 20 grams daily for five to seven days, reach saturation faster but produce the same end result as a steady lower dose over three to four weeks. Either approach works.
Timing matters less than consistency. Taking it post-workout with a carbohydrate source improves uptake slightly, but the difference is marginal compared to simply taking it every day. Hydration is worth monitoring, creatine draws water into muscle cells, which is part of how it works, and total daily water intake should increase modestly.
Cycling off creatine is not necessary. The body's own production does not shut down permanently. After stopping supplementation, muscle stores return to baseline within four to six weeks.
The real story of creatine in India is not about a dangerous supplement hiding in plain sight. It is about a well-documented, inexpensive, and effective compound that a generation of gym-goers has been taught to fear by a chain of trainers who never read a study. The myths have lasted longer than they should because the people repeating them are trusted, and the people who could correct them rarely speak up in the same language.
