A polysomnography test can diagnose what is keeping you from sleeping properly, but most people either don't know it exists or assume it takes forever to arrange. In India, the process is faster than it looks, if you know which specialist to call first, what to ask about home-based testing, and how to read the referral chain before you enter it.

Start With a Pulmonologist or Neurologist, Not a General Physician

The referral chain is where months get lost. A general physician will often send you to an ENT, who may send you back, who may then suggest a cardiologist if snoring is involved, and somewhere in that loop the sleep study never gets ordered. Go directly to a pulmonologist or a neurologist and ask specifically for a sleep evaluation. Both specialties can order a polysomnography in India, and both typically have the referral pathways already set up. In metro cities, a sleep medicine clinic run by a pulmonologist is often the fastest single stop.

Understand What You Are Actually Asking For

A sleep study, formally called polysomnography, or PSG, records brain activity, oxygen levels, heart rate, breathing patterns, and limb movements while you sleep. It is the standard diagnostic tool for obstructive sleep apnoea, periodic limb movement disorder, REM sleep behaviour disorder, and certain insomnias that do not respond to basic treatment. The test does not hurt and requires no preparation beyond avoiding caffeine and alcohol on the day. What it does require is one full night, either in a lab or at home with a portable device. Knowing what you are asking for makes the conversation with a specialist faster and prevents you from being routed into a general wellness consult instead.

The Home Sleep Test Is Available in India and Cuts the Wait Significantly

For straightforward suspected obstructive sleep apnoea, the most common reason adults are referred, a home sleep test, or HST, is now an accepted first-line diagnostic in Indian sleep medicine practice. The device is smaller than a paperback, records oxygen saturation and airflow through the night, and is collected the next morning. Several hospitals in Mumbai, Delhi, Bengaluru, Hyderabad, and Chennai offer them on a rental or supervised-loan basis. The Indian Journal of Sleep Medicine has published on the validity of home-based testing for OSA diagnosis in Indian populations, and the Indian Sleep Disorders Association recognises it as appropriate for uncomplicated presentations. If the specialist suggests an in-lab study for a suspected straightforward OSA case, it is reasonable to ask whether an HST would serve the same diagnostic purpose. For complex cases, suspected narcolepsy, parasomnias, or cases where cardiac monitoring is needed, the in-lab study is not replaceable.

What the In-Lab Wait Actually Looks Like and How to Shorten It

At government teaching hospitals with sleep labs, AIIMS Delhi, PGI Chandigarh, NIMHANS Bengaluru, the wait for an in-lab PSG can run from six weeks to several months depending on the season and referral load. At private hospitals and standalone sleep clinics, the same test is typically available within one to three weeks. The cost at private facilities ranges widely by city and centre; asking for the all-inclusive rate covering the technician, scoring, and physician interpretation prevents a bill that arrives in parts. Many health insurance policies in India now cover polysomnography when ordered by a specialist for a documented clinical indication, check your policy's diagnostic test clause before paying out of pocket, because reimbursement is more common than most patients expect. If cost is the constraint and a government facility is the preference, ask the specialist to mark the referral urgent where the clinical picture supports it; this is standard practice and not an unreasonable request.

After the Report: What Happens Next and What to Expect

A sleep study report scores your sleep architecture and, if sleep apnoea is present, gives an Apnoea-Hypopnoea Index, the number of breathing interruptions per hour. An AHI under five is normal; five to fifteen is mild; fifteen to thirty is moderate; above thirty is severe. The report alone does not end the process. A follow-up appointment with the interpreting physician is where treatment is discussed, CPAP therapy for moderate to severe OSA, positional therapy or an oral appliance for mild cases, or further testing if the diagnosis is unclear. That appointment should be booked at the same time as the study, not after the report arrives, because the gap between report and follow-up is the second place where months quietly disappear. The sleep study is a diagnostic tool, not a treatment. Bring the report to a specialist who can act on it.
