Your BMI tells you how much you weigh relative to your height. Your VO2max tells you how long you are likely to live. A landmark study in JAMA Network Open found that low cardiorespiratory fitness is a stronger predictor of mortality than smoking, diabetes, or obesity. Here is what the number measures, why it matters, and how aerobic training moves it.

The Number Doctors Are Now Calling a Vital Sign

In 2016, the American Heart Association formally recommended that cardiorespiratory fitness, measured by VO2max, be recorded as a clinical vital sign alongside blood pressure and resting heart rate. The recommendation followed a meta-analysis of 33 studies covering more than 100,000 patients, published in Circulation, which found that low fitness was associated with a higher risk of all-cause mortality than hypertension, high cholesterol, or type 2 diabetes individually. VO2max is the maximum volume of oxygen your body can consume and use during intense aerobic exercise, expressed in millilitres of oxygen per kilogram of body weight per minute (mL/kg/min). A score of 35 mL/kg/min for a 40-year-old Indian man puts him in the "below average" category. A score above 50 puts him in the top tier, with mortality risk dropping sharply at each step up.
The BMI comparison is not flattering to BMI. A 2018 study in JAMA Network Open tracked 122,007 patients over 23 years and found that low cardiorespiratory fitness carried a higher mortality hazard ratio than obesity, smoking, or hypertension. A person with a high BMI but good VO2max had significantly better survival outcomes than a lean person with poor fitness. The researchers described low fitness as "a stronger predictor of mortality than any other established risk factor."

What VO2max Is Actually Measuring

The number captures a chain of physiological events: how efficiently your lungs pull oxygen from air, how effectively your heart pumps oxygenated blood, how quickly your muscles extract and use that oxygen, and how well your mitochondria convert it to energy. A high VO2max means every link in that chain is working well. A low one means at least one link is failing, and the failure tends to be systemic rather than isolated.
This is why VO2max correlates so tightly with longevity. The organs and systems it measures, cardiac output, lung capacity, mitochondrial density, vascular health, are the same ones that deteriorate in the diseases that kill most Indians: cardiovascular disease, type 2 diabetes, and metabolic syndrome. The Indian Council of Medical Research's 2023 national burden of disease data placed cardiovascular disease as the leading cause of death in India, responsible for roughly 28% of all mortality. Most of those deaths involve the same systems VO2max tests.
VO2max also declines with age at a predictable rate, about 1% per year after 25 in sedentary individuals. Regular aerobic training slows that decline to roughly 0.5% per year. Over 30 years, that gap compounds into a meaningful difference in functional capacity and disease risk.

Why BMI Misses What VO2max Catches

BMI was designed in the 1830s by the Belgian statistician Adolphe Quetelet as a population-level tool to describe the "average man." It was never intended as an individual health metric. It cannot distinguish between muscle and fat, does not account for fat distribution, and has well-documented accuracy problems for South Asian populations specifically.

South Asians, including Indians, carry metabolically active visceral fat at lower BMI thresholds than Western populations. A study published in the Lancet Diabetes and Endocrinology found that Indian adults show insulin resistance and cardiovascular risk at BMI levels that would be classified as "normal" by standard WHO cutoffs. The Indian Council of Medical Research has recommended lower BMI thresholds for Indians, 23 as overweight, 25 as obese, precisely because the standard scale misclassifies risk in this population.
VO2max sidesteps the body composition problem entirely. It measures what the body can do, not what it weighs. A person who walks briskly for 45 minutes five days a week and has a VO2max of 42 mL/kg/min carries a fundamentally different mortality risk than someone who weighs the same but whose VO2max sits at 28, regardless of what either person's BMI says.

How to Improve Your VO2max Without a Lab

A formal VO2max test requires a metabolic cart, a treadmill, and a sports science lab. The closest consumer approximation is a GPS running watch with heart rate monitoring, brands like Garmin and Polar estimate VO2max from pace and heart rate data, with accuracy within 5% of lab measurements in peer-reviewed validation studies.

Training to raise it follows a clear principle: work at intensities that push your cardiovascular system close to its ceiling, then recover. Three methods with strong evidence behind them:
1. High-intensity interval training (HIIT): Four minutes at 90 to 95% of maximum heart rate, followed by three minutes of easy recovery, repeated four times. A 2007 study by Helgerud et al. in Medicine and Science in Sports and Exercise found this protocol increased VO2max by 7.2% in eight weeks, more than any other training format tested.

2. Zone 2 aerobic base work: Sustained effort at 60 to 70% of maximum heart rate for 45 to 90 minutes. This builds the mitochondrial density and fat oxidation capacity that supports high-intensity work. Most endurance athletes and longevity researchers, including Dr. Peter Attia, recommend 80% of weekly training volume in this zone.
3. Tempo runs or threshold cycling: Sustained effort at the pace or power output you can hold for roughly an hour. This raises the lactate threshold, which determines how long you can sustain a high percentage of your VO2max before fatigue sets in.
For most Indians with desk jobs and limited time, two HIIT sessions and two Zone 2 sessions per week, about 3.5 hours total, produce measurable VO2max gains within 12 weeks.

The Fitness Number That Compounds

A VO2max of 40 mL/kg/min at 40 becomes a VO2max of 34 at 55 if you do nothing. The same starting point, with consistent aerobic training, becomes 37 or 38, a gap that translates, in the Circulation meta-analysis data, to a 15 to 20% reduction in all-cause mortality risk. That difference is not cosmetic. It shows up in whether you develop heart failure at 65 or 75, whether you recover from a respiratory illness in five days or three weeks, whether you can climb three flights of stairs without stopping at 70.
BMI tells you something about your body's composition at a single moment. VO2max tells you something about the trajectory your body is on, and unlike BMI, it responds directly and measurably to what you do next.
