Eczema in Indian adults keeps flaring despite topical creams because the trigger is often inside the gut, not on the skin. Chronic dysbiosis, inflammatory diet patterns, and a disrupted microbiome drive the immune response that shows up as a rash. Understanding this gut-skin connection changes what treatment actually needs to address.

The skin is the symptom, not the source

Eczema, clinically called atopic dermatitis, affects roughly 10 to 30 percent of children and a significant share of adults in India, according to data published in the Indian Journal of Dermatology. Most adults manage it with moisturisers, topical steroids, and antihistamines. Many cycle through flares for years without resolution. The reason is straightforward: the skin is where the inflammation surfaces, but the immune dysregulation driving it often originates in the gut.


The gut houses roughly 70 percent of the body's immune cells. When the microbial balance in the intestine is disrupted, a condition called dysbiosis, the immune system shifts into a chronic low-grade inflammatory state. That inflammation does not stay local. It travels through the bloodstream and manifests in the skin as redness, itching, and the characteristic weeping patches of atopic dermatitis.


What dysbiosis looks like in the Indian diet context

Indian adults carry specific dietary risk factors that Western eczema research often misses. The widespread reliance on refined wheat (maida), excess refined sugar in mithai and packaged foods, and the near-total disappearance of traditionally fermented foods from urban diets has changed the gut microbiome in ways that matter for skin health.


Fermented foods like homemade curd, kanji, and idli-dosa batter made with a proper overnight ferment introduce Lactobacillus strains that support the gut lining. When those are replaced by commercial probiotic drinks with minimal live cultures, or cut out entirely, the diversity of the microbiome drops. A 2019 study published in JCI Insight found that patients with atopic dermatitis showed significantly lower populations of Roseburia and Faecalibacterium prausnitzii, two bacterial strains associated with anti-inflammatory short-chain fatty acid production, compared to controls without eczema.



Antibiotic overuse compounds this. India's rates of antibiotic prescription, including self-medication without prescription, are among the highest globally. Each course strips the gut of bacterial diversity. Repeated courses across childhood and early adulthood leave a microbiome that struggles to regulate immune signalling, which is exactly the mechanism behind atopic dermatitis flares.


The leaky gut mechanism

Dysbiosis degrades the integrity of the intestinal lining. Tight junctions between gut epithelial cells loosen, allowing partially digested food proteins and bacterial fragments to pass into the bloodstream. The immune system reads these as foreign invaders and mounts a response. In people with a genetic predisposition to atopy, which includes a large portion of South Asian adults, this response skews toward a Th2-dominant immune profile, the same profile that drives eczema, asthma, and allergic rhinitis.



This is why many Indian adults with eczema also report food sensitivities, bloating, or irregular digestion. The gut and skin complaints are not separate problems running in parallel. They share the same upstream failure.


What actually shifts the pattern

Addressing gut health alongside topical treatment changes outcomes in a way that topicals alone cannot. The evidence base is building. A 2021 meta-analysis in the British Journal of Dermatology reviewed 39 randomised controlled trials and found that probiotic supplementation, particularly with Lactobacillus rhamnosus GG and Bifidobacterium strains, produced a statistically significant reduction in SCORAD (Severity Scoring of Atopic Dermatitis) in adults.



Practically, this means three things. First, reintroducing genuine fermented foods: curd made at home from live cultures, fermented rice water, or a daily serving of properly prepared idli or dosa batter rather than the instant mix. Second, reducing the dietary load that feeds dysbiosis: refined carbohydrates, ultra-processed snacks, and excess sugar. Third, being deliberate about antibiotic use and discussing gut restoration with a physician after any course, not as an afterthought, but as part of the treatment plan.


None of this replaces dermatological care. Topical steroids still control acute flares. Moisturisation still protects the skin barrier. The point is that without addressing the gut, these interventions manage the symptom on a loop.



Reading the flare differently

A flare after a wedding season, three weeks of rich food, late nights, alcohol, and disrupted meals, is not random. A flare after a round of antibiotics for a throat infection is not coincidence. When the gut is under stress, the skin reports it. Indian adults who have been told their eczema is chronic and unmanageable often have not been asked what their gut is doing.


The dermatology appointment that only looks at the skin is working with incomplete information. The gastroenterologist who sees bloating and irregular digestion and does not ask about skin is doing the same. Atopic dermatitis in adults sits at the intersection of both, and the treatment that works is the one that acknowledges that geography.


Skin that keeps breaking out despite every cream applied to it is asking a question about the interior. The answer is rarely on the surface.
