Most senior cats show no outward signs until kidney or thyroid disease is already advanced. Routine bloodwork changes that. A simple feline screening panel, creatinine levels, SDMA, and T4 thyroid values, can flag serious trouble two to four years before your cat looks sick. Here is what those numbers mean and when to start testing.

Why cats are biologically wired to hide illness

Cats conceal pain and weakness as a survival instinct inherited from solitary wild ancestors who could not afford to appear vulnerable. A senior cat with early kidney failure will eat, groom, and sit in your lap looking perfectly fine until roughly 75 percent of kidney function is already gone. That threshold, three-quarters of the organ destroyed before a single clinical sign appears, is the central problem that bloodwork exists to solve. A 2016 study published in the Journal of Veterinary Internal Medicine confirmed that SDMA (symmetric dimethylarginine), a kidney biomarker now included in standard feline panels, detects chronic kidney disease an average of 17 months earlier than creatinine alone. In some cats, that gap stretched to 24 months. For a species that lives 14 to 18 years on average, 17 months is a meaningful window.


What the kidney panel actually measures

A standard senior feline kidney screen includes four values: creatinine, BUN (blood urea nitrogen), phosphorus, and SDMA. Each tells a different part of the story.Creatinine rises when the kidneys can no longer filter waste efficiently, but it is a late signal, it only climbs after significant nephron loss. BUN follows a similar pattern and is further affected by diet and hydration, making it an unreliable standalone marker. Phosphorus elevation becomes clinically significant in later-stage disease and is used more to guide treatment than to catch early trouble.SDMA is the early-warning instrument. It is released by all nucleated cells in the body at a constant rate and cleared almost exclusively by the kidneys. When kidney clearance drops even modestly, SDMA rises, and it does so before creatinine budges. IDEXX Laboratories, which developed the SDMA assay, recommends establishing a baseline SDMA value in cats as young as seven so that any upward trend over subsequent years is visible against that individual cat's normal, not just against a population reference range. A cat whose SDMA sits at 11 µg/dL at age seven and climbs to 16 µg/dL at age ten has moved meaningfully, even if 16 is technically within the published normal range of up to 18 µg/dL.


The thyroid panel and why it complicates everything

Hyperthyroidism is the most common endocrine disorder in cats over ten years old, affecting roughly 10 percent of that population according to data published in the Journal of Feline Medicine and Surgery. The thyroid gland, when overactive, floods the body with T4 hormone, accelerating metabolism, driving weight loss despite a ravenous appetite, and raising heart rate. Most owners notice the weight loss first, often months after the thyroid has already been running hot.The T4 blood test, total thyroxine, is the primary screen. A result above 4.0 µg/dL in a cat showing clinical signs is diagnostic. But the relationship between hyperthyroidism and kidney disease creates a diagnostic trap that a single-organ approach misses entirely: elevated thyroid hormone artificially increases blood flow to the kidneys, which raises their filtration rate and temporarily suppresses creatinine and SDMA into normal ranges. A cat with both hyperthyroidism and underlying kidney disease will look kidney-healthy on bloodwork, until the hyperthyroidism is treated. Once T4 drops back to normal, the kidneys' true function is unmasked, and what looked like healthy kidney values may suddenly read as Stage 2 or Stage 3 chronic kidney disease.This is why treating the thyroid without simultaneously tracking kidney values is incomplete medicine. The International Society of Feline Medicine recommends a kidney recheck two to four weeks after initiating any hyperthyroidism treatment, precisely because this unmasking effect is predictable and common.


When to start and how often to screen

Most veterinary guidelines place the start of senior screening at age seven, which corresponds to roughly the mid-40s in human terms. The American Association of Feline Practitioners recommends full bloodwork, including a complete blood count, biochemistry panel with SDMA, urinalysis, and T4 thyroid value, once a year from age seven, and every six months from age eleven onward.Urinalysis deserves specific mention. Urine specific gravity, the measure of how concentrated the urine is, can signal kidney trouble even when blood values are still normal. A cat producing consistently dilute urine, specific gravity below 1.035, is a cat whose kidneys are losing concentrating ability, and that finding alone warrants closer monitoring.For Indian cat owners, access to SDMA testing has expanded significantly through veterinary diagnostic labs in metro cities. Vets at larger urban clinics in Mumbai, Bengaluru, and Delhi now routinely include SDMA in senior panels, though some smaller clinics still run older panels without it. If your vet's standard panel does not list SDMA, ask specifically for it, it is available as an add-on through most major diagnostic networks.



Reading the results with your vet

Numbers without trend are only half the picture. A single creatinine reading of 1.8 mg/dL is mildly elevated but not alarming on its own. The same reading, compared against a baseline of 1.1 mg/dL two years earlier, tells a different story, the kidneys have lost ground, even if the current value sits within the broad reference range.This is why baseline bloodwork at age seven matters as much as the annual screens that follow. The baseline is the ruler everything else gets measured against. A cat whose creatinine, SDMA, and T4 are all documented at age seven gives you a personalised reference point that population averages cannot provide.When values trend upward, early intervention options open up: prescription kidney diets that reduce phosphorus and protein load, phosphate binders, subcutaneous fluids administered at home, and in the case of hyperthyroidism, methimazole medication or radioactive iodine therapy. None of these are cures, but each slows progression measurably. The International Renal Interest Society's staging system for feline chronic kidney disease, now used by vets worldwide, grades disease from Stage 1 (no clinical signs, abnormal biomarkers only) through Stage 4 (end-stage). Cats caught at Stage 1 or early Stage 2 through routine screening have the longest and most comfortable windows ahead of them.The bloodwork does not prevent the disease. What it prevents is the disease running unopposed for years in a cat who still looks fine sitting on your kitchen counter.
